 $$  Your payment must be submitted with this registration form.  Please make checks payable to Skyline Agility Inc.  In the event that you are “wait listed”, your check will be held until final confirmations.  Checks will be returned if your entry is unsuccessful.  A $20 fee will be charged for any checks returned with insufficient funds

 Non Club Members.  It is recommended that you submit your registration form as early as possible.  Applications will be “Wait Listed “ in the order of arrival.  As soon as the Club Member priority date is reached you will be notified of acceptance. 

 Cancellation and Refund Policy.  A full refund will be given if cancellation is made more than 6 weeks prior to the seminar.  After that time, a refund will only be given if we are able to find a replacement from the “wait list”.  If no one is “wait listed”,  participants may find their own replacement.
